Here's what to expect upon submitting the form:
- You will receive an email confirmation acknowledging that we have received your request for service
- One of our service scheduling specialists will confirm your appointment with a phone call during normal business hours to verify we have all necessary information
- Within 24 hours of the visit from our service technician, one of our service scheduling specialists will follow-up via phone to confirm a technician is on the way

*If a purchase order is required, acceptance of this contract is limited to the terms stated herein. All other terms on Customer’s documents that are additional to or inconsistent with this form are objected to and rejected and shall be deemed a material alteration.
SERVICE FEE: $85.00 per technician, per hour
Services are billed $85 per service technician hour including travel. The minimum charge for our services is $175. The minimum charge includes evaluating the condition of the roof so repairs can be accurately estimated. In the event the estimated cost of repairs exceeds the agreed upon Service Not To Exceed value we will provide an estimated cost to complete the repair. Should you decline to proceed with the repairs, or in the event where repairs cannot be performed, you agree to pay the minimum service fee of $175. Should you agree to proceed with the estimate the minimum service fee will be applied to the cost of the repair. Rates apply to normal business hours. Nights and weekend rates may differ.
Customer agrees to make payment no later than 10 days from the date of the invoice. If Superior Services RSH, Inc. is not timely paid, Superior Services RSH, Inc. shall be entitled to interest at the rate of 1.7% percent per month plus costs of collection, including attorneys’ fees.

The repairs performed by Superior Services RSH, Inc. are based on Superior Services RSH, Inc. visual inspection and investigation of the area of the reported problem. Superior Services RSH, Inc. will not perform infrared or nuclear scans or destructive testing or water testing before commencing work unless specifically requested and paid for by Customer. Unless otherwise agreed, Superior Services RSH, Inc. will only perform a visual inspection and cannot guarantee that no additional problems will be discovered once repairs begin. Customer acknowledges and understands that after Superior Services RSH, Inc. commences its work, new or additional problems may be uncovered that were not discovered by Superior Services RSH, Inc. visual inspection and that the price and time to complete the repairs may be increased as a result of such problems. Customer further acknowledges and agrees that Superior Services RSH, Inc. is not responsible for damages or leaks caused by existing conditions or existing sources of leakage simply because Superior Services RSH, Inc. started work or performed repair work.
NO WARRANTIES, EXPRESS OR IMPLIED, ARE PROVIDED BY SUPERIOR SERVICES RSH, INC. IN CONNECTION WITH SERVICE WORK UNLESS OTHERWISE AGREED IN WRITING. UNLESS OTHERWISE AGREED, ALL SUCH WARRANTIES, INCLUDING IMPLIED WARRANTIES OR MERCHANTABILITY AND FITNESS FOR A PARTICULAR PURPOSE, ARE EXPRESSLY EXCLUDED AND DISCLAIMED. SUPERIOR SERVICES RSH, INC. SHALL NOT BE LIABLE FOR INCIDENTAL OR CONSEQUENTIAL DAMAGES OR INJURIES INCLUDING ALLEGED DAMAGE TO THE BUILDING OR ANY COMPONENTS OR CONTENTS THEREOF, MOLD, MILDEW, LOSS OF USE, RENTAL INCOME OR PERSONAL PROPERTY.
